Acetaminophen vs Ibuprofen for Fever: Which Works Better?

For reducing fever alone, both work well and the difference between them is modest. Ibuprofen tends to bring temperature down slightly more and lasts longer per dose. Acetaminophen is safer for more people.

The right choice usually comes down to your other symptoms and your medical history.

When ibuprofen is the better choice

  • Fever with significant body aches — the anti-inflammatory effect helps
  • Sore throat, where inflammation is the source of pain
  • Menstrual pain, dental pain, joint pain
  • You need longer coverage between doses, particularly overnight

When acetaminophen is the better choice

  • You have kidney disease
  • You have stomach ulcers or a history of GI bleeding
  • You take blood thinners

  • You are pregnant
  • You are dehydrated from vomiting, diarrhea, or poor intake
  • You are over 65 — NSAID risks rise with age

That dehydration point is often overlooked. When you are properly unwell and not drinking much, ibuprofen adds strain to kidneys that are already under pressure. Acetaminophen is the safer default in that situation.

Can you take both?

Yes. They can be combined or alternated for most adults, and the combination works better than either alone. Full dosing schedules in taking ibuprofen and Tylenol together.

Correct dosing

Acetaminophen: 500–1,000 mg every 6 hours. Maximum 3,000 mg per day as a safe working limit for healthy adults, 4,000 mg absolute ceiling. Lower if you drink alcohol or have liver disease.

Ibuprofen: 200–400 mg every 6 to 8 hours with food. Maximum 1,200 mg per day over the counter.

Neither should be used for fever for more than three days without medical advice, or for pain beyond ten days.

The overdose risk that is easy to miss

Acetaminophen appears in more than 600 US products. NyQuil, DayQuil, Theraflu, Excedrin, and many prescription pain medications all contain it. People exceed the daily limit without realising because they are counting only the Tylenol.

Should you treat a fever at all?

Fever is part of the immune response, and there is no strong evidence that reducing a mild fever prolongs illness in adults. But equally, there is no benefit to suffering through one.

The reasonable approach: treat when you feel bad enough that it interferes with rest, fluids, or sleep. Do not treat a temperature of 99.5°F just because the number looks wrong.

What matters more than the number is how you look and feel. A person at 103°F who is alert and drinking is in better shape than someone at 100.5°F who is confused. See when a fever needs the ER.

Non-drug measures

Fluids, light clothing, a room around 68°F, and rest. Skip cold baths and alcohol rubs — they cause shivering, which raises core temperature and makes things worse.
